Terms and Conditions

Each guest will be required to complete consent and healthcare forms prior to the initiation of any treatment. The forms will be emailed after an appointment has been scheduled and will request online signatures. Please complete all forms prior to arriving for your appointment.

SCHEDULING APPOINTMENTS


Prior to receiving any service, guests are required to complete a consultation. During your consultation, you will discuss your past medical history, concerns and treatments that you are interested in. You will be provided recommendations based on this information and will be able to schedule your appointment if deemed appropriate at that time. In order to schedule an appointment either a debit card or credit card is required. A nonrefundable reservation fee is required for every appointment scheduled. The reservation fee includes your consultation.

CANCELLATIONS


Al appointments that are cancelled before 24 hours of appointment will be given a one-time courtesy to reschedule when canceled. The nonrefundable reservation fee will be used for the new appointment time. Appointments that are not rescheduled within 24 hours of appointment will forfeit the nonrefundable reservation fee.
